La PTA du Var Ouest organise en collaboration avec la Maia du bassin hyérois, le CLIC Hadage et le CIAS de la Vallée du Gapeau, des réunions de cellule par secteur.
Objectif de ces réunions
Ces temps d’échanges entre professionnels ont pour objectif de permettre à chacun d’évoquer des situations pour lesquelles le maintien à domicile est, ou a été, problématique. Il s’agit de :
- Repérer et analyser les points de rupture dans les parcours des patients.
- Apporter des réponses grâce à l’expertise de chacun des participants.
- Rechercher, le cas échéant, le ou les acteurs les mieux à même de traiter la situation identifiée.
Fonctionnement
Une Charte est en place afin de définir le fonctionnement de ces réunions et les règles d’échanges et de partage d’informations.
De nombreux professionnels participent à ces réunions de cellule qui ont conduit à des échanges productifs. Elles ont permis d’identifier des pistes d’amélioration relatives aux points de ruptures repérés sur la situation présentée.
Ces réunions sont également un excellent moyen de rencontrer des acteurs de terrain de la psychiatrie, du handicap, de la personne âgée. Ainsi elles permettent de créer des liens professionnels nécessaires à l’amélioration des parcours de santé des personnes prise en charge.
